As a parent of a child with a disability or chronic health condition, you know that working with and communicating with many health, education, insurance and social service providers can be difficult.  While stressful,  it is a very important task in ensuring your child's  medical and non-medical needs are met.
Below are resources that may help you in your role as your child's Care Coordinator:

The Care Coordination Plan Worksheet should be a "snapshot" of what is happening with your child now, and can be updated regularly.  The plan can be shared with those providing care for your child.
