Life Launch: FREE Transition Planning Workshop Series

South Dakota Parent Connection is sponsoring a five-session transition workshop series at no cost for youth with disabilities

(ages 16-20 years of age) and their parent/guardian!

WHY PARICIPATE

YOUNG PARTICIPANTS:

(ages 16-20)

· Gains skills for using your voice.

· Understand the importance of making connections and building relationships.

· Build goals for the future based on your strengths, values, and interests.

· Learn about how to make a plan for moving to the future.

 

PARENTS & FAMILY MEMBERS:

· Learn how your role with your young adult is changing.

· Get tips for teaching your young adult how to speak up.

· Gain tools and strategies for making positive plans for the future.

· Learn how to support your young adult to navigate successfully through agencies and support systems.

The five-session workshop series will give an overview of resources for transition planning. It provides the young adult and her/his family information to help navigate systems and move forward in a positive, productive direction.
